connecting rod clearance

what would cause the rod to eat into the side of the crank? 100-150 miles and there is a knock. scat rods on a fresh 10/10 crank. was told rod journal side clearance was .017 on a couple. but now 1 is at .035 what would cause the rod to want to pull to one side? and wear the bearing on the other?
 
Bent?
In backwards?
Twisted? [Pin c/l and brg end c/l not parallel]
When the crank was reground, was the journal width correct? [What was the side clearance on ALL 6, when the engine was assembled??]
All 6 rods the same width at the big end?
All rods have the correct chamfer to match the crank?
