Your wideband looks erratic, mine shows a smoother log. I would turn the boost down to what it was when the knock began in your log until you figure it out. 6 degrees of knock at 24 PSI is not a good combination.

Do you have logs from when it was newer? I’ve heard people say replace the sensor yearly but I’ve been running mine for years. I think it depends on where in the exhaust they are located. Closer to the turbo isn’t going to last very long. Mine is at the end of the downpipe.

I would expect that will need to be replaced. Mine is mounted at the other end right by the test pipe. Yours is getting a lot of heat there.
Probably time for a new sensor

You will need to check your fuel pressure as well...The knock retard happened at the highest boost level so probably is not false knock....Is that chip you have a wide band tracking chip??
 
Your log should look more like Scott's. If it's always looked like that something is wrong with the wiring or the wb box.
 
You probably want to turn off wideband correction until you get it sorted out.
