Anyone ever see this happen before?

This thermostat was brand new last Summer. No idea how this happened.

Luckily I pulled it while putting in a new water pump.
 
I have seen that a couple of times. That happens when the rod isn't captured in the cup. Better designed thermostats have a ball on the end of the rod which is crimped into the cup to keep it from pulling out.
 
That is strange , I just replaced a stuck open t stat yesterday , Its not bad but no fun to replace
